Investigation Summary
At approximately 9:15 a.m. on August 25, 2008, an employee was working on a pneumatically powered shear press, PEXTO, Model Number 137-K, Serial Number 3/58, to cut a fabric material sheet, 18-in. by 24-in., to make 1.5" wide strips. He had made about 5 cuts before a strip got jammed behind the blade of the shear. As the employee tried to remove the jammed material from the blade, he inadvertently depressed the foot pedal and his right index finger got caught between the blade and the table of the shear, which caused amputation of his index finger. The employee was not hospitalized.
