Employee's Toe Is Amputated by Falling Forklift Load

At approximately 9:30 a.m. on July 13, 2008, Employee #1, a helper, was working at a steel forging business. He was wearing heavy safety toed shoes. A driver of a Linde manipulator forklift, Number 836, Model Number H50D, Serial Number H2X394T03738, grabbed an OD tooling with the jaws of the manipulator. The driver backed up to allow Employee #1 to apply lubrication to the OD tooling. The OD tooling is 34-in. in diameter ring, made of high-carbon steel, weighing 595 pounds. As the driver rotated the OD tooling, the grip loosened due to a hydraulic malfunction and the tooling fell off the jaws. The tooling hit the floor, bounced and landed on Employee #1's left foot. He suffered an amputation of his left toe and was hospitalized.
