Employee's Four Fingers Are Amputated by Moving Chain

At about 7:30 a.m. on June 23, 2008, Employee #1 was greasing a sprocket and chain drive on a manure spreader truck, Mohlang Brush, Colorado, XHD-22, License Number 4D91761. Employee #1 was using a grease gun with his right hand. His left gloved hand was caught in the moving sprocket and chain drive, causing amputation of his four fingers. Employee #1 underwent three surgeries and was hospitalized for two weeks.
