Employee's Hand Is Amputated Between Forklift and Racks

At approximately 7:30 a.m. on November 6, 2007, an employee was backing out a Crown stand-up fork lift, Model Number RR5225-45, Serial Number 1A312208, Equipment Number 115, to position herself to pull out empty pallets from a case pack module in Aisle C20, in the warehouse. As she was backing the fork lift up, she noticed that the fork lift would be hitting the rack system in the back. So, she tried to reverse the forklift but could not control it. She panicked and stuck her left hand out onto the rack system, Rack 111A2, at which time her hand got caught and pinned between the rack frame and left rear vertical member of the forklift causing serious injuries to her left hand. The employee was hospitalized and treated for an amputation of hand.
