Employee's Leg Is Fractured Between Forklift and Bollard

At approximately 11:00 p.m. on December 2, 2007, Employee #1, a freight handler, was operating a standup counterbalance forklift, a Raymond Model Number R40, C40TT, Serial Number R40-07- 13469, at a loading dock. He was not certified or trained to operate this forklift. He was going in reverse and came too close to a group of bollards. Fearing he was going to crash into a bollard, he stepped off the forklift. The forklift crashed into one of the bollards and his left lower leg was caught between the forklift and one of the bollards. Employee #1 was hospitalized with a fractured leg.
