Event: 12/18/2007
Employee Is Injured When Struck by Falling Steel Mold

At 9:45 a.m. on December 18, 2007, Employee #1 was taking apart a 960 pound steel mold. The mold was resting on a steel table approximately three feet high. As Employee #1 was taking apart the mold, Employee #1 felt a vibration and the mold began to fall. Employee #1 attempted to catch the mold with his right arm. However, the mold was too heavy. Employee #1 attempted then to get out of the way of the falling mold. The mold fell onto the cement floor, striking Employee #1's right toes. Employee #1 was transported to Inland Valley Medical Center, in Wildomar, California. Employee #1 sustained a broken right arm that required a plate and six screws in his arm, as well as the amputation of Employee #1's right big toe and the one next to it.
