Investigation Summary
At approximately 8:09 a.m. on December 7, 2007, Employee #1, a maintenance mechanic from the maintenance repair service, was working by the roadway located Northeast within the facility. Employee #1 dismounted the truck, Ram 3500 CA 14746, California License Plate Number 6U63646, and while he was attempting to stop his truck from rolling further down the slope, the vehicle slammed into the parked flatbed vehicle, ID 45424, license Number 4BC4229, which was 40-ft long, 8-ft wide, and 57-in. high. Employee #1 was pinned between the tongue of the flatbed and the cabinet behind the cabin of the service truck and sustained asphyxia. Employee #1 was killed.
