Employee's Finger Is Amputated When Caught Between Molds

At approximately 2:00 p.m. on October 16, 2007, Employee #1 was trying to clear a jammed part from the plastic molding Forming Machine, Brown Machine Model Number R-223, Serial Number S-7722, by using air pressure to release the part from the mold. He was using a small air hose nozzle with his right hand, having his index finger on the nozzle. As he tried to lift the upper mold by using a toggle switch, he accidentally pushed the switch in the wrong direction and brought the mold down on his index finger. His finger was crushed between the molds, causing it to be amputated. Surgery was performed on his right hand, and he was hospitalized for three days.
