Employee Caught Between Forklift,trailer, Sustains Fractures

At approximately 11:04 a.m. on October 4, 2007, Employee #1 was operating the large forklift. The forks were suspended approximately 7-ft above the ground. A two-axle trailer chassis was attached to the forks with a chain draped over one of the forks. The chain was draped evenly on both sides. As Employee #1 started to lift the trailer chassis, the chain shifted a little to the right side of the fork. Employee #1 got down from the forklift and walked to the area between the front right side of the forklift frame and the suspended trailer chassis. While Employee #1 was standing in this area, the hook on the left side of the chain came undone from the chassis. The chassis swayed and pinned Employee #1 between the forklift wheel and wheel of the chassis. The chassis came to rest on the ground at this time. Employee #1 sustained unspecified fractures and was hospitalized for nine days.
