Investigation Summary

Investigation Nr: 202447967
Event: 10/26/2007
Employee Burns Hand When Caught in Hot Plate Section

At 9:00 p.m. on October 26, 2007, Employee #1 who worked for a manufacturer of corrugated packaging, was rethreading the single face web into the hot plate section of the SCM short press. A coworker went back to assist Employee #1 in rethreading the web from the bridge through the preheaters and into the hot plate section. After Employee #1 told the coworker to start up the corrugator and to get ready to chop out the bad portion, the coworker hit the green (start) button and was headed back toward the knife controls to chop out the bad portion. Employee #1 stated that he reached over with his right hand to give the liner a pull because it was not threading. The right side of the broken bridge caught Employee #1's right hand and drew it into the hot plate section between the web and the liner. Employee #1's right hand became burned upon contact. Employee #1 was transported to the Arrowhead Regional Medical Center where he was admitted. Employee #1 was hospitalized for approximately five days.
