Investigation Summary
On August 2, 2006, Employee #1 was working for his employer, a distributor of marble and quartz. Employee #1 and a forklift operator were loading two marble slabs at a time onto a flat bed truck. Each slab was 10-ft by 5-ft by 0.75 in. and weighed 800 lbs. They had already loaded approximately 70 slabs. Employee #1 was holding the slabs to prevent them from swinging as the forklift moved to the truck. The forklift sped up unexpectedly and ran over his left foot. He was hospitalized for eight days for unspecified fractures to his foot.
