Investigation Summary
At approximately 22:00 p.m. on July 13, 2007, Employee #1 and a coworker were using a cherry picker, (Crown Model Number ZP3200, Serial Number 1A190046), to go up the third level to fix a crooked pallet with merchandize on the rack. The coworker was using a body belt, and Employee #1 was wearing neither a harness nor a body belt. When Employee #1 stepped on the grate on a rack Number 10, being on the third level at the height of 16 ft 9 in. in the warehouse, the grate got displaced from the rack. Employee #1 fell down to the concrete floor together with the grate, the pallet, and the merchandize. Employee #1 sustained lacerations of his right upper arm and ankle. Initially, that was a non-serious injury but due to infection, Employee #1 had a second surgery of his right arm on October 12, 2007.
