Investigation Summary
At 6:45 p.m. on July 17, 2007, Employee #1 was cutting three plywood sheets (4-foot by 8-foot by 0.25-inch) to make 4-foot by 4-foot pieces using a vertical panel saw (Type-1225, Serial Number 602/0-005-(2000). The employer is engaged in the manufacturing of marker boards. Employee #1 had cut about 50 pieces before the accident. After making one cut, he removed one half of the plywood sheet with his two hands. As he tried to remove the other half, he hit the control lever accidentally. The saw moved up and the blade struck Employee #1's left thumb, causing the amputation of his thumb. Employee #1 was hospitalized for 13 days.
