Investigation Summary
At approximately 8:10 a.m. on July 9, 2007, an employee was cutting a 2.5-in. wide by 4-in. long by 0.9375-in. thick plywood laminated piece on a 10 inch-Tannewitz Quality, type U circular table ripsaw. While cutting a 0.125-in. wide notch in the small wooden piece by placing on its side, it kicked back. The employee's s left hand inadvertently contacted the moving saw blade, causing amputation of the ring and middle fingers and laceration of the left little finger. The employee was taken to the US Healthworks, Chino, CA for treatment. The clinic referred the employee to the Orthopedic Medical Group and Rehabilitation Center, Glendora, CA for surgery. Surgery was performed the same day, and the employee was discharged at 11:00 p.m.
