Investigation Summary
At approximately 10:30 a.m. on June 15, 2007, an employee of Regional Steel Corporation was greasing a chain on a rebar bender, Arnold Rebar Bender, identified as Number 14, Serial Number 44. The employee would apply some grease on the chain then depress the machine's foot pedal in order to move the chain. While the employee was applying the grease, he inadvertently depressed the foot pedal activating the machine. The chain moved and pulled his right hand into the pinch point between the chain and the small sprocket. The employee was transported to Metropolitan Industrial Medical Clinic then to Arrowhead Regional Medical Center. The employee was hospitalized and treated for a finger amputation.
