Employee's Leg Is Crushed Between Pallet Jack and Shelf Rack

At approximately 12:44 p.m. on May 15, 2007, Employee #1 was operating an empty Crown powered pallet jack, Model Number PE4500, in the shipping area of a warehouse, when it ran over a wood chip and began to skid to the right. As the employee attempted to steer the pallet jack to the left to compensate, he lost control and it struck the racks of a horizontal shelf. The employee's left leg became pinned between the pallet jack frame and the shelf and was crushed. He was transported to the Arrowhead Regional Medical Center in Colton, CA for treatment. He required surgery and was hospitalized for four days.
