Investigation Summary
At approximately 10:30 p.m. on June 5, 2007, Employee#1 was operating the energized rubber and paint mixer. The employer manufactures rubber mats made form recycled rubber. The facility is enclosed and has two rubber and paint mixers. At the time of injury Employee #1 was mixing rubber chips with various glues and color additives by adding materials to the mixer hopper. Employee #1 stated he recalls that the accident happened when he tilted the (mixer) hopper to pour the mixture into the plastic container. Employee #1 stated as he tilted the mixer hopper, the edge of the hopper hit him in the mouth and his left hand went inside the hopper and contacted the energized mixer blades. Employee #1 was standing on the floor level and was facing the mixer hopper when the accident occurred. The elevated paint and rubber mixer hopper housing the mixer blades is open on the top. The opening measured 27-in. wide by 22-in, long. The most vertical position of the mixer blade measured approximately 6-in. from the rim of the mixer hopper. The mixer blades are attached to a horizontal rod within the confines of the paint and rubber mixer hopper. Access to the elevated hopper is by two fixed steps. Employee #1 sustained open fractures of the index, long and ring fingers on his left hand when it came in contact with the mixer blades on the energized paint and rubber mixing machine. Employee #1 was hospitalized.
