Employee's Finger Is Amputated in Rollers of a Machine

On May 22, 2007, Employee #1 was operating a roller conveyor. He noticed rollers were jammed by pieces of broken glass. He grabbed and shook the metal guard to loosen the glass but it did not work. The end section of the roller conveyor system stopped running. He lifted the metal guard of the chain and sprocket and started to remove the broken glass with his right hand until the rollers started to move again. He attempted to put back the guard when the tip of his glove made contact with the chain and sprocket mechanism. His hand was pulled in and twisted. A coworker pushed the emergency stop button and called for help. Employee #1 tried to free his hand, but was unable too. Maintenance unscrewed the bearing housing to lift the roller but it did not free his hand. They reversed the direction of the roller manually, freeing his hand. Employee #1 sustained a right index finger amputation up to the middle of his knuckle and was hospitalized.
