Investigation Summary
At approximately 9:00 a.m. on May 1, 2007, Employee #1 was working on a Royer-16F, sand aerator machine. He was in the green sand conditioning department. He was trying to push a stalled inclined conveyor of the aerator machine under power with a shovel. The power source to the sand aerator machine was not deenergized or disengaged prior to pushing the conveyor with the shovel. The conveyor started running and his hand slipped from the shovel and was caught between the running conveyer and the left side rail. Employee #1 was hospitalized with lacerations to his right index finger and thumb.
