Investigation Summary
At approximately 11:00 p.m. on April 11, 2007, Employee #1, the operator of an automatic palletizer (Mollers, PLS, identified as Number 5) climbed onto the forming table of the palletizer in an attempt to correct a deformed layer of bagged product in the forming table and pusher area. Employee #1 did not lockout the equipment prior to climbing onto the forming table. Either Employee #1 or the now correctly formed bags tripped a sensor that activated the transfer section of the palletizer. The transfer section continued to cycle. Employee #1's right leg was pinned between the moving transfer section and the stationary tamping plate. Three coworkers and a shift supervisor responded to assist Employee #1. In process of freeing Employee #1 a coworker activated the palletizer and as a result Employee #1's right ankle was fractured. Employee #1 was transported to hospital where she underwent surgeries and she was discharged on April 20, 2007.
