Investigation Summary
At approximately 3:00 p.m. on April 25, 2007, Employee #1 was unloading material stacked on a pallet from a truck (Bob Tail Truck, License Number 7U93008) by using a fork-lift (Crown forklift, Serial Number 1A166273). As Employee #1 was backing up the fork-lift out of the trailer (Bob Tail Truck, License Number 7U93008) the truck moved forward and the forklift tipped over and fell between the back of the truck and the dock platform. Employee #1 was pinned between frame of the forklift and the dock floor and sustained a serious fracture which required five surgeries and hospitalization for five days.
