Investigation Summary
At approximately 7:20 a.m. on April 11, 2007, Employee #1 was cutting a V-ditch (swale) form out of a 2-in. by 12-in. by 3.5-in. board using a Skilsaw (Model Number HD77M, 7.25-in., 4400 rpm). Employee #1 held the stock in his left hand and the saw in the right hand to make a cut. During cutting, the saw blade jumped and contacted Employee #1's left thumb causing amputation. Employee #1 was hospitalized.
