Investigation Summary
At approximately 12:09 p.m. on May 7, 2007, Employee #1, an equipment operator, operates a Belling Machine. The Belling Machine manufactures PVC pipes, which are extruded out onto a turntable. The turntable is automatically raised with the PVC pipes, and they are transferred to a storage rack. Employee #1 was standing next to the storage rack waiting for the PVC pipe to transfer from the turntable to the storage rack. Once the PVC pipes are on the storage rack, Employee #1 pushed a lever down to activate two metal rod stoppers. The two metal rod stoppers then go to the down position and the PVC pipes are transferred to the unitizing table. Once the PVC pipes were on the unitizing table, Employee #1 removed his hand from the lever. The lever is supposed to return to its original position and then the two metal rod stoppers return to the up position. However, the lever was stuck and did not return to its original position. The two metal rod stoppers remained in the down position. Employee #1 was standing near the storage rack to stencil the PVC pipe, when another PVC pipe, 20.66 ft long, 18-in. in diameter and weighing 657 pounds was extruded from the Belling Machine onto the turntable. It then moved to the storage rack and rolled off due to the two metal rod stoppers stuck in the down position. Employee #1 was struck in the head by the falling PVC pipe and died.
