Employee's Finger Tip Is Amputated When Crushed

At approximately 8:30 a.m. on March 21, 2007, Employee #1, a foreman, was working at a construction site. A new BAC cooling tower Model Number BAC 3299A, a 8.5 ft by 18 ft by 8.33 ft high metal structure was set on its foundation in the mechanical yard. There was difficulty in aligning one of the 0.875-in. diameter mounting holes so that an anchor bolt could be installed. A coworker used a 36-in. bull pry bar to inch the tower into position, while Employee #1 used his right index finger to check the line meant. The cooling tower moved approximately 0.5-in. and Employee #1's finger tip was severed.
