Investigation Summary
On January 4, 2007, an employee and three coworkers had installed roofing felt on a reverse A roof with a 4-in 12 slope. Located on each side of the roof were 4 skylight openings that measured approximately 4-ft long by 4-ft wide. There were total of 8 skylights that were covered with the roof felt and marked with orange spray paint. The employee stepped backwards and fell through the inadequately covered skylight opening. He fell 30 feet 8 inches to a concrete floor below and landed on his face. The employee was pronounced dead on the scene. The coworkers that were interviewed at the site stated that they were utilizing full body harnesses with lanyards attached to rope grabs that were attached at the peak for fall protection. One coworker stated that the roof anchors were removed to allow for the last row of roof felt to be installed that went on the peak of the roof and covered both sides of the roof.
