Employee Is Caught in Machinery and Killed

Employee #1 and a coworker were looking at a mulch coloring machine to determine why the machine was not operating properly. The machine consists of a hopper which is fed mulch using a front end loader, and augers which mix the mulch with color. Color is fed into the machine automatically. Employee #1 and the coworker were standing on the ground. Employee #1 had just turned the machine on. The coworker was looking at the machine and had taken a step or two to his right, then turned back to speak to Employee #1. Employee #1 was not in sight. The coworker called his name and started to duck under the machine to go to the other side to look for Employee #1 when he heard the motor for the machine sound different. The coworker immediately pushed the emergency stop button and continued looking for Employee #1. The coworker did not see Employee #1 on the ground, so he climbed a ladder on the machine and looked into the hopper, where he saw Employee #1 caught in the augers. The coworker immediately called for help but Employee #1 had already died from the injuries received. When Employee #1 was removed from the machine, his position indicated that he had most likely fallen into the machine head first. Employee #1 was killed.
