Investigation Summary

On November 10, 2006, two employees had assisted a customer to load stone products into a truck's trailer. While the customer was driving the truck onto a state highway, Employee #1 attempted to control traffic. He was struck by a vehicle and killed. Employee #1 had not been instructed or asked by the customer to assist his entry onto the highway. No traffic devices or high visibility clothing was used during the incident. No employee training in traffic control had been provided prior to the incident.
