Employee Is Run over by Forklift, Sustains Toe Amputations

On August 21, 2008, Employee #1 and a forklift were traveling in the same direction and in close proximity. The forklift driver turned the wheel to gain some distance between the forklift and Employee #1. The forklift struck Employee #1 in the shoulder with the back of the forklift and ran over his left foot. Employee #1 suffered the amputation of four toes and was hospitalized.
