Investigation Summary
On February 29, 2008, Employee #1, a production worker for Swiss American Sausage Company, was operating the Urschel dicer when a jam occurred. Employee #1 reached into the machine from the bottom with the right hand and made contact with the blades. Employee #1 suffered amputation of the index, middle and ring fingers and partial thumb of the right hand.
