Employee Crushes Hand When Trailer Axle Plate Drops

On October 9, 2007, Employee #1, a welder/assembler, lifted a trailer using a 5-ton hoist and alloy chain rigging. Employee #1 attempted to place a spacer between the axle mounting plate and the trailer. The alloy chain rigging was incorrectly attached to a D-ring on the trailer. While the trailer was hoisted, the D-ring flipped up, dropping the trailer approximately 4 to 6 inches. Employee #1's right hand was between the axle mounting plate and the trailer, which resulting in a crushing injury to his fractured hand. Employee #1 was hospitalized.
