Investigation Summary
On September 18, 2007, Employee #1, a Foreman for Employer, attempted to unjam a pallet that had gotten stuck inside the Hess Board Stacker machine. Employee #1 disengaged the overhead stacker unit, but did not disengage the conveyor. Employee #1 entered the machine and the conveyor auto-started, trapping the left lower leg of Employee #1, resulting in a fracture and hospitalization.
