Worker Dies in Fall from Roof

On October 6, 2006, Employee #1 was lifted on a maintenance platform that was attached to the forks of a forklift truck. He traveled to the roof which was approximately 15 feet high to retrieve a piece of plastic that was hanging from an air-conditioning unit. The forklift driver positioned the platform approximately one foot away from the roof's edge, so that the employee could easily access the roof. Employee #1 jumped from the platform onto the roof, lost his balance and fell approximately 15 feet to the ground. Employee #1 was taken to a local hospital where he died the following day.
