Investigation Summary

Investigation Nr: 201954831
Event: 01/20/2006
Employee Fractures Finger In Machine

On January 20, 2006, Employee #1 was operating a teeth-hardening machine on a rack manufacturing line, rack Line Number 2. The machine hardened the teeth section of a rack used in the steering mechanism for automobiles. Hardening occurred through a 200-ampere, 300-volts heater for the teeth section. Employee #1 was hand loading the machine with a rack when the machine cycled. His hand was caught in the center holding device, clamps, and the gate, movable barrier device. A coworker stopped the machine before heat was applied. The holding device crushed Employee #1's right index finger with possible nerve damage and caused lacerations and contusions to his right hand.
