Investigation Summary
Two employees were working in the mill room around the Number 9 mill. The employees were working on the front end of the mill under a suspended conveyor system call a Mill Loader. The Mill Loader is suspended approximately 6 ft above the floor. The mill had a total of 10 Mill Loaders in the plant and all were suspended from the ceiling structure in the building. The Mill Loaders had been in the facility for approximately 18 years. While the two were working, the two front supports for the Mill Loader at Number 9 mill broke and it fell to the floor. Employee #1 was crushed and fatally injured while the other employee received a superficial cut to his head.
