Investigation Summary
At approximately 11:15 a.m. on March 1, 2007, workers were installing eight 10 ft grounding rods into the ground in the immediate vicinity of a lumber stacker building. Employee #1 was applying pressure to a hammer gun while attempting to drive the ground rod into the soil. Upon hitting rock the Sky Trak and the basket used as a standing platform from which to apply additional downward pressure were repositioned. Employee #1's back was to the hydraulically driven unit of lumber on the out feed conveyor rail while he was facing the basket. He was looking down at the grounding rod and or hammer gun when his upper torso was struck from the rear and driven into the basket. His body was entrapped between a stack of 16 ft long lumber and the basket of the Sky Trak lift. He suffered massive crushing injuries to his chest and abdomen and died.
