Investigation Summary
Employee #1 was working from an elevated walk board 9 feet above the floor, when he became ill and sat down on the board. Coworkers stated that he then laid down on the board and would not respond when they called his name. They heard him exhale loudly and then roll off the board and fall to the floor. He was killed. After further investigation, it was suspected that the employee suffered heart failure.
