Employee's Hand Is Amputated in Rollers

On July 6, 2009, Employee #1 was applying an air pipe to the material being compressed at line Number 2 compression rolls. There were a total of three rollers at the scene, with the two upper rollers rotating at opposite directions, and a third roller located at the bottom rotating counterclockwise. The third roller was seldom used during the normal operation, but someone apparently activated it at the time of accident. Employee #1's right hand was caught between the second and the third rollers and was amputated. Upon the investigation, it was determined that the injury was caused by lack of fixed or self-adjusting barrier.
