Employee's Fingers Are Amputated in Machinery

At about 6:20 pm on July 1, 2009, Employee #1 was feeding sheet metal to a roll former, a shop machine, Number K5, located on the east side of the facility. The machine was shop built and was in operating service for quite some time. No Model Numbers or Serial Numbers were identified on the machinery. Employee #1 observed that the sheet metal he just fed into the front of the machine was misaligned. He attempted to straighten the sheet metal by reaching in and bumping the sheet metal with his left hand. Although Employee #1 was wearing gloves that covered his hands and forearms at the time of the incident, the fast-moving sharp sheet metal cut right through the gloves, causing amputation of the mid, index and small fingers of his left hand. Employee #1 was hospitalized. Employee #1 was trained prior to the accident not to reach into machinery, while it was in motion.
