Investigation Summary
At approximately 4:56 p.m. on April 10, 2009, Employee #1 was having his flatbed truck loaded with approximately 24-ft 6-in. long bundled aluminum alloy extrusion material. The material was being loaded by a forklift driver. During the loading process, there were several attempts to properly load the material on to the flatbed truck. Employee #1 was not satisfied by the way the material was being placed on the top of the truck and requested the forklift drivers to unload and reload the materials. Employee #1 climbed on top of the flatbed truck and materials and guided the forklift drivers in placing the materials on the truck bed. Employee #1 pushed the bundles to check the stability of the load. The aluminum alloy extrusion materials that were being placed on top of the flatbed truck inadvertently fell over and struck Employee #1. Employee #1 sustained multiple broken bones including his neck, back, arms and legs as well as head injuries. Employee #1 was hospitalized and had six surgeries.
