Investigation Summary

Investigation Nr: 202455309
Event: 04/20/2009
Employee's Finger Tip Is Amputated While Installing Counter

At approximately 8:30 a.m. on April 20, 2009, Employee #1, a finish carpenter, was working at a custom cabinet and counter top manufacturing and installation company. He was preparing and gluing a wooden counter top joint in the attempt to install a counter top. He trimmed to fit and inserted biscuit joints on a medium density fiberboard laminated counter top that was 81-in. long by 23-in. wide by 1-in. thick. He positioned the counter top close to an existing edge of a counter top he had installed earlier. He leaned the counter top against a wall and diagonal building support and steadying the counter top with his left hand, with the two joints approximately 4-in. apart. He was spreading glue on the joint and the counter top slipped from the building support causing the two counter top sections to rapidly slide together. His right fourth finger tip was caught between the counter tops, resulting in a crushing open fracture. Employee #1 was hospitalized and his finger tip was amputated.
