Employee Inhales Gas When Tank Explodes

On March 16, 2009, Employee #1 was entering a delivery vehicle to retrieve materials while two others were outside the van within proximity. The delivery vehicle was using three composite CNG tanks of about 3,000 PSI Rated capacity, measuring 16-in. in diameter by about 55-in. in length. It was being refueled in the rear driveway of the business by virtue of a cascade bottle fueling array adjacent to the vehicle. The tanks were manufactured by Brunswick Company, have a Model Number of RA30A16-055AB and are fitted with a trade name "TuffShell" foam liner on the shoulders of the tanks. The tanks exploded, subjecting Employee #1 and the coworkers to the gas. Employee #1 and coworkers were hospitalized. The employer removed the delivery trucks from service and subsequently replaced the tanks on all their trucks voluntarily after the incident. NOTE: The narrative describes three employees being hospitalized but there is only one injury line in the IMIS accident summary.
