Investigation Summary

Investigation Nr: 310494547
Event: 02/24/2009
Employee Amputates Finger in Machine

At approximately 11:03 a.m. on February 24, 2009, an employee was adjusting roofing material to allow the roll winder machine to continue rolling the roofing material. The machine that the employee was working on was a left hand ejected electric drive roll winder and core inserter manufactured by Reichel and Drews Inc. The employee suffered an amputation to the distal phalanx of his right ring finger. The cause of the accident was due to improper guarding of the roll winder machine he was working on. It was determined that there had been a power surge in which production was briefly interrupted. This caused the material in the production line to sink into the cooling water. The wet material caused winding issues and moved the roofing material out of alignment because of its wet state. At this point, the employee had switched the machine into manual mode to adjust the material. While adjusting the material, the role winder started, causing the apron, guiding plate, to pinch his right hand between it and the spindle. The employee was hospitalized and treated for a finger amputation.
