Employee's Leg Is Fractured in Pallet Jack Wreck

At approximately 5:30 p.m. on November 18, 2008, Employee #1, an order selector, was operating a powered ride on pallet jack attempting to take a trash bin to the shipping dock. She was operating the powered ride on pallet jack, Number 46, eastbound along the main travel aisle. As she passed through the doorway adjacent to aisle 21, she observed a coworker on a powered ride on pallet jack driving southbound from aisle 20 and beginning to turn northbound into aisle 21. Fearing a collision, Employee #1 took evasive action, lost control of her vehicle and collided with a four inch diameter steel pipe filled with concrete, embedded in the floor. She sustained a fractured right lower leg and was hospitalized.
