Investigation Summary
On April 18, 2008, an accident occurred at Jack Rubin & Sons, a material handling supply company in Compton, California. Employee #1 was a shop foreman and had worked for the company for 32 years and held the position of shop foreman for 12 years. On the afternoon of April 18, 2008, Employee #1 was using a hand grinder to shorten a steel button that was on a wire rope. When Employee #1 turned on the hand grinder, the blade on the hand grinder disintegrated, causing a piece of the blade to strike Employee #1 on his right thigh. Employee #1 was taken to the St. Francis Medical Center where he was treated for his injuries.
