Employee Falls from Ladder and Fractures Leg

At approximately 8:30 a.m. on June 5, 2008, Employee #1 was working in the Finance Department file room located in the Graphics Department basement area. Employee #1 was positioned on a rolling ladder filing Finance Department documents. Beneath the ladder were two approximate 4-ft wide pieces of cardboard. As Employee #1 came down the ladder to the second and third steps, the rolling ladder shifted back towards the door causing her to fall to the ground level floor. Employee #1 sustained a fractured femur which required complete hip surgery. Employee #1 was discharged on July 12, 2008.
