Investigation Summary
On April 28, 2008, Employee #1 was a machine operator at a contaminated soil remediation facility. He was cleaning a screw auger conveyor to remove dirt that had built up on the conveyor walls. Employee #1 was standing in a 24-in. screw auger conveyor, removing dirt with a digging bar, a scraper, and a small shovel. Employee #1 informed his supervisor that the cleaning job was finished. The supervisor observed the work area through a window in the control room. Believing Employee #1 was standing clear of the screw auger conveyor, the supervisor started the machine. Employee #1 was injured when his left foot was crushed between the screw auger and the conveyor walls. He was hospitalized for the amputation of his foot.
