Investigation Summary
On February 25, 2008, Employee #1 was working for a cold storage-refrigerated warehouse business. He was using a battery operated industrial truck equipped with an overhead guard. He was pulling a pallet of cheese from the storage racks when he dislodged one of the cross bars of the rack causing the cheese to fall. He stepped out of his truck to avoid the cheese and was struck in the leg. He was hospitalized with a fractured leg.
