Investigation Summary
At approximately 12:00 p.m. on January 15, 2008, Employee #1 and a coworker finished emptying cardboard trash from a trailer truck into a dumpster, located on an outside roadway area, south of Building A. Employee #1 positioned herself on a ramp at the back of the trailer truck approximately 3 ft high and held on while the coworker drove forward. While the trailer truck rounded the corner of Building A, Employee #1 fell off the vehicle and hit the ground. Employee #1 sustained a contusion, lacerations, and soft tissue injuries to the head. Employee #1 was transported to the Los Angeles County Hospital and received brain surgery. She remained there for approximately three weeks and was then transported to White Memorial Hospital to receive additional brain surgery.
