Investigation Summary

Investigation Nr: 202454328
Event: 11/07/2007
Welder loses his leg in explosion of O2 cylinders

On November 11, 2007, employee # 1, a 20 year old male welder with The Gomes Enterprises LLC DBA A to Z Metals was working at the aluminum sorting and torching yard. Employee # 1 was cutting an aluminum scrap metal piece 6 feet by 4 inches, to remove non-aluminum piece using oxygen and propane cylinders as part of the torch cutting equipment. Employee # 1 was working on this aluminum scrap metal part that was on top of a pile of other aluminum scrap metal parts. It is normal practice for employees to torch cut metal parts on top of other metal parts that are placed in piles. When employee #1 was torch cutting there was an explosion and fell backwards facing up. Employee # 1 right leg was severed leading to an amputation of his right leg about mid-calf down. The cause of the explosion was the presence of oxygen cylinders found under the aluminum scrap metal pile, one of which exploded. After the accident, the injured employee was driven to Saint Francis Medical Center where his leg was amputated.
