Investigation Summary

Investigation Nr: 202454104
Event: 10/22/2007
Employee Amputates Fingerswhile Adjusting Machine

At approximately 3:00 a.m. on October 22, 2007, Employee #1 was operating a Valmet Rotomec Printing Press, Serial Number 1132 (Machine Number14), in the printing area of the facility. This machine transfers ink onto plastic bags which run through a series of rollers and a transfer plate located on one of the rollers. The portion of the machine where the incident occurred incorporated the use of a sanitizing blade which presses against a roller next to transfer plate roller thereby keeping the ink from building up on the roller and inadvertently transferring onto the product. Unwanted spots began appearing on the bags as they ran through the machine which required the adjustment of the sanitizing blade. While the machine was energized and running Employee #1 crawled under a portion of the machine's frame to access the blade. After Employee #1 had adjusted the blade he turned to get out when his right hand became caught in two in-running rolls near the sanitizing blade pulling his hand into the machine. Employee #1 had been trained to de-energize and tag out the machine. Employee #1 sustained an unspecified amputation and was hospitalized.
